REQUEST MEDICAL RECORDS

In order to get a copy of your medical records from Tri-State Memorial Hospital, you must submit a signed and dated Information Release Form. Please click here to download a PDF of the “Information Release Form” that you can fill out on your computer. You may hand-deliver or fax this form to the medical records office at 509.758.3566.

Please make sure to include the following information to help ensure quick processing of your request:

• Name (or patient’s name)

• Date of birth

• Social security number

• Date of service and treatment at Tri-State Hospital

• Your address or the address to which you are sending your records

• Your telephone number

• Your signature

Requesting Medical Records of a Deceased Patient

Please contact the Medical Records Department at 509.758.5511. You may need to provide additional documents in order to obtain a copy of the records.
