The Case Management and Discharge Planning Department of Tri-State Memorial Hospital is comprised of a Registered Nurse (RN) Case Manager and a Social Worker (BSW) Discharge Planner who help by coordinating your care from the time you enter the hospital until you are discharged.

Our department helps to arrange the needed care and services you might need after your hospitalization.  These services may include: Home Health or Hospice Care, Skilled Nursing Facility Care, Durable Medical Equipment, IV Infusion Therapy, Community Resource Planning, and assist in creating an Advance Directive for Health Care.

We are available to answer any questions you may have in regards to your anticipated needs after hospitalization.  The RN Case Manager also works with your insurance company to keep them updated on your status, and provide regular updates to your physicians in regards to your status and needs.  The Social Worker is also available to help provide patients and families with emotional support as needed.
